Radioactive substances
(Refer to Section of the RSC Manual)
The RSC needs to approve, record and monitor all radioactive substances which are held on University grounds. To assist with this, each Facility has been provided with a quadruplicate booklet to track all radioactive substances which have been used and stored in each laboratory.
To obtain RSC approval for the Purchase/Acquire of a Radioactive Substance, you need to:
- complete the white-coloured copy of the “Application to Purchase/Acquire Radioactive Substances and Notice of Disposal” form in the quadruplicate booklet.
- forward the form to the Presiding or Executive Officer of the Radiation Safety Committee, together with a copy of the Purchase Requisition form (only if you are purchasing the substance), for approval;
- After consideration, the Presiding/Executive Officer shall endorse the form and forward a copy to you with an RSC Approval Number
If you are purchasing the substance, upon receipt of an approved copy of the form, forward it- together with the purchase requisition - to the Purchasing Officer, Division of Financial Services.
Upon the Arrival of a Radioactive Substance, you need to:
- complete the blue-coloured copy of the “Application to Purchase/Acquire Radioactive Substances and Notice of Disposal” form in the quadruplicate booklet.
- forward the form to the Presiding or Executive Officer of the Radiation Safety Committee so that the database may be updated; and
- remove a self-adhesive label from the rear of the quadruplicate form book, complete the appropriate details and attach it to the substance’s container prior to use/storage.
On Disposal, you need to:
- complete the pink-coloured copy of the “Application to Purchase/Acquire Radioactive Substances and Notice of Disposal” form in the quadruplicate booklet.
- forward the form to the Presiding or Executive Officer of the Radiation Safety Committee so that the database may be updated.
Disposal of a radioactive substance also includes the decay of the substance to below its reportable level or forwarding the substance to another facility within or external to the University.
Finally, the lemon-coloured copy of the “Application to Purchase/Acquire Radioactive Substances and Notice of Disposal” form shall remain in each book for the radiation facility records.
