With Adobe Acrobat you can create electronic forms from scratch or you can convert your paper forms to Adobe PDF files.

Creating forms in Acrobat

The most common way to construct a form would be to use Microsoft Word as a template. If you are going to make an online form that is not primarily going to be printed, its advisable to construct the form in landscape mode as opposed to portrait. This will facilitate for better screen reading.

For this demonstration I have made the form in Adobe Illustrator for clarity, but you could have used Word or Excel. You are reminded at this stage to save your original Word document etc. In the same place as a copy of the completed PDF that you distribute. The most common problems arise when you need to make changes to that form and you cant find the original document that you made it from.
