- Police: Please note the name of the police officer, date and time of reporting and event number given by police.

Complete the attached claim form and return to the insurance officer (provide link to contact us page). Please note that all thefts, losses or damages arising out of a single event are one single claim. An internal excess of $2000 is applied to all property claims.
General and Products Liability
Upon the happening of any incident likely to give rise to a claim, advice must be forwarded to the insurance officer (provide link to contact us page) together with originals of all correspondence received from a third party and accompanying accounts. Complete the attached claim form (link to claim form general) and return to the insurance officer (provide link to contact us page).
Under no circumstances must liability be admitted either verbally or in writing.
Professional Liability, Professional Liability Malpractice, Director & Officers
Upon the happening of any incident likely to give rise to a claim, advice must be forwarded to the insurance officer (provide link to contact us page) together with originals of all correspondence received from a third party and accompanying accounts. Please note that these policies are underwritten on a "claims made" basis. This means that these policies require all claims and any fact, situation or circumstance that may result in a claim, to be notified to the Insurer within the Period of Insurance. The Insured must not have had any prior knowledge of the fact, situation or circumstance before the Period of Insurance. The policy will not provide cover in relation to:
- Events that occurred prior to the retroactive date of the policy;
- Claims made after the expiry of the period of cover even though the event giving rise to the claim may have occurred during the period of cover;
- Claims notified or arising out of facts or circumstances notified (or which ought reasonably to have been notified) under any previous policy or indemnity arrangement;
- Claims made, threatened or intimated against you prior to the commencement of the period of cover;
- Facts or circumstances of which you first became aware prior to the period of cover, and which you knew or ought reasonably to have known had the potential to give rise to a claim under this policy;
- Claims arising out of circumstances noted on the Application or Renewal Form for the current period of cover or on any previous proposal or renewal form.
Motor Vehicle
In the event of an accident the driver is to:
- Stop and exchange name and address of the other party involved and obtain details of their vehicles insurance policy
- Record registration number and make of other vehicle
- Ensure that exact location of the accident is noted, together with times and conditions of road (eg slippery following heavy rain, dry etc)
- Report accident to Police if required to do so by State Motor Traffic Law or By-Law
- Notify the relevant campus Transport Officer as soon as practical after the accident
- Driver to complete a vehicle accident claim form and give to Transport Officer
- Any documentation eg Letters of Demand should be forwarded immediately to the Insurance Officer (link to contact us)
- If necessary you may provide the third party with the University's insurance details which are:
