In order to provide an appropriate service to students, the DLO needs to know about any assessments by health practitioners etc as to the nature of the student’s disability and the likely impact on their study. As the Guidelines for the Disability Standards for Education Standards 2005 state,

When considering an adjustment for a student with a disability, a provider is entitled to information about the student’s disability and individual requirements if that information is directed towards:

Any confidential information provided to education providers for the purposes of making adjustments should not be disclosed except for the purposes of the adjustment or in accordance with a lawful requirement.

At CSU, the personal or health information provided to the Disability Liaison Officer (DLO) is protected by the Privacy and Personal Information Protection Act 1998 (NSW). Any information provided by you to the DLO should be accurate and is given voluntarily. The information is collected by the DLO for the purpose of supporting you in your studies and will only be disclosed to relevant CSU staff for this purpose. This information may be recorded in a file and will be destroyed in accordance with the General Disposal Authorities as set by State Records NSW.

Your information will not be supplied to any person or organisation outside the University for any purpose unless authorised by you except where the University may be legally required to do so.

You may request the DLO to allow you to view your file at any time and request in writing that information be updated if it is incorrect. Relevant information to support this request will need to be provided.

If you are dissatisfied with the way the DLO has handled your personal information you may apply to have the matter reviewed by lodging a formal application with the CSU Ombudsman.
