Professional Placement Host Forms
Copies of professional placement forms for host library/organisation supervisors to complete are available below for download and print:

Host supervisors are asked to complete a Supervisor's Report at the end of the placement period. We ask that you give your considered opinion based on your interaction with and observation of the student over the period of the placement. Your assessment of the student on placement is a valued contribution to the determination of the student's grade. Please note that the Supervisor's Report may be completed in consultation with the student on placement.
