Abstract:

This subject is offered concurrently with the supervised clinical practice to provide an understanding of the sonographic approach to the field of obstetrics and gynaecology. Students will undertake appropriate scanning techniques to image the female pelvis including all trimesters of pregnancy. Knowledge of embryology processes will underpin delivery of this subject. Students will be encouraged to use an analytical approach to identify and understand the clinical questions.

Enrolment restrictions:

Available only to students enrolled in the PostgradDipMedUltrasound or the MMedUltrasound course or at the discretion of the Course Coordinator.

Objectives:

Upon successful completion of this subject, students should:

* be able to undertake the obstetric examination of a pregnant woman across all trimesters and determine the: - length of gestation; - foetal well-being; - presence and type of any gross fetal abnormalities; - presence of any maternal complications.
* demonstrate competence in the trans-abdominal and trans-vaginal female pelvis ultrasound examination;
* be able to perform these examinations in a safe and ethical manner providing due care for the patient;
* be able to present sonographic images of obstetric and gynaecological examinations demonstrating a logical and methodical approach to scanning and communicate findings appropriately.
* be able to recognise a range of normal and abnormal image appearances of the female pelvis and fetus and understand the contributing role and nature of pathologic and embryologic processes; and
* demonstrate the capacity to critically examine ethical issues related to obstetrics and gynaecology and reflect on how these issues impact on professional practice.

Syllabus:

The subject will cover the following topics:

1. Embryology: Gametogenesis. Fertilisation & 1st week; 2nd week; 3rd week; 4th week-nervous system; Peripheral nervous system; embryonic folding (cavities, mesenteries, lungs); Heart; vasculature; gastrointestinal tract; urinogenital tract; Limbs; Head, neck, eyes and ears; Brain and cranial nerves; integumentary; Placenta. 2. Female Pelvis: trans-abdominal scanning. Trans-vaginal scanning. Normal menstrual cycle. Infertility. Assisted fertilization techniques. Laboratory tests and complementary procedures. Common abnormalities and pathologies. 3. First trimester of pregnancy: Gestational duration, heartbeat, number of foetuses and maternal ovaries and uterus. Screening for some early signs of gross foetal abnormalities. Chorio villi sampling and amniocentesis. Nuchal Translucency Screening. Trans vaginal scanning. Pregnancy testing. 4. Second trimester of pregnancy: Gestation, number of foetuses, placental site and the foetal 18-20 week morphological assessment. Biometry. 5. Third trimester of pregnancy: Foetal growth, well being, physiology, position and amniotic fluid volume. Trans labial scanning. 6. Issues related to obstetrics and gynaecology: Detection and presentation of foetal abnormalities. Abnormality disclosure. Sonographer to patient communication. Confidentiality. Sonographer reporting findings. 'Non-diagnostic' scanning. 3D and 4D scanning.
