Abstract:
Building on NRS421 Chronic Care Nursing Practice 1, this subject will enable students to develop a holistic understanding of the issues related to chronic condition self-management from a lifespan and biopsychosocial perspective of the person with chronic conditions. Through case studies, personal research and reflection on practice, students will enhance their understanding of nursing interventions used to support people with chronic conditions, their interactions with health systems and their use of self management techniques and models. Students will develop an understanding of the barriers to and facilitators of effective self-management for and by the person.

Enrolment restrictions:
Students enrolled in Postgraduate Certificate in Chronic Care Nursing Practice or Master of Nursing courses. Subject to course coordinator approval, registered nurses with at least 12 months professional experience may enrol as an associate student.
Objectives:
Upon successful completion of this subject, students should:
Be able to examine common physiological, mental health and developmental chronic conditions.
Be able to assess, plan, provide and critically evaluate advanced nursing interventions in the management for patients/clients with common chronic conditions.
Be able to explore and critically discuss the interrelationship between chronic conditions and the impact of co-morbidity on health.
Be able to assess, plan, provide and evaluate advanced nursing interventions in the provisions of management of patients/clients in primary, secondary and tertiary health care settings with co-morbid chronic conditions.
Syllabus:
The subject will cover the following topics:
Module 1: Common Chronic Physiological Diseases
This module will examine nursing interventions for people with common chronic conditions such as cardiovascular disease, chronic respiratory disease, diabetes and chronic kidney disease.
Module 2: Chronic Mental Health Conditions
This module will examine nursing interventions for people with chronic mood disorders and chronic psychotic disorders. It will also explore the link between other chronic conditions and depression.
Module 3: Developmental Disability
This module will examine nursing interventions for people with genetic, congenital, prenatal, perinatal and postnatal contributors to and the long term complications of developmental and intellectual disability.
Module 4: Co-Morbidity of Chronic Conditions
The module will examine the complexity of co-morbid chronic conditions and the provision of nursing interventions. It will also include an examination of the impact of living with multiple chronic conditions on the older person.
