Abstract:

This subject provides students with an understanding of diabetes mellitus and a thorough knowledge of the impact of diabetes on the lower extremity, from an individual perspective through to a global perspective. It also provides students with a theoretical knowledge of the assessment and management of chronic wounds. The combination of these two areas equips the students to work with clients to reduce morbidity and to prevent lower extremity amputations.

Nil - Students are expected to have knowledge of diabetes, biomechanics, and an understanding of the human body in health and disease.

Objectives:

Upon successful completion of this subject, students should:

Be able to:

evaluate the aetiology, pathophysiology, complications and management of diabetes mellitus;

critically discuss issues in the pathophysiology, diagnosis and management of the patient with diabetes with respect to the lower extremity;

critically evaluate the aetiology, principles and practice, pathophysiology, complications and management of chronic wounds;

appraise the integrated team approach to client care with respect to diabetes and wound management; and

analyse the physical, emotional and social impact of diabetes/chronic wounds on the client, carers, family and society.

Syllabus:

The subject will cover the following topics:

Aetiology, pathophysiology, clinical management, complications, impact on individuals, communities, nationally and globally of diabetes Neuropathies associated with diabetes (theory, aetiology, pathophysiology, clinical presentation, medical and podiatric management) Charcot Neuroarthropathy (theory, aetiology, pathophysiology, clinical presentation, medical and podiatric management) Vasculopathies associated with diabetes (theory, aetiology, pathophysiology, clinical presentation, medical and podiatric management) The effect of diabetes on the musculoskeletal system and the management of those effects, including pressure redistribution strategies and footwear prescription Wounds and Infections in the lower extremity of a person with diabetes, including neuropathic ulceration Issues in the diabetic foot. (Concepts of the at risk/high risk foot, identification of the high risk foot, classification systems, access to services etc.) Client education/self care model/multidisciplinary teams Concepts of wound healing - Physiology and pathophysiology of chronic wounds and wound healing Wound assessment - principles, practice and issues Wound management - principles, practice and issues Wound infection, theory, identification and management Nutrition and wounds Neuropathic ulceration Arterial ulceration Venous ulceration Wound management products Adjunct strategies in wound management (pressure offloading, compression, hyperbaric oxygen, footwear, orthoses, total contact casting, crutches, wheelchairs etc.) Wound management teams
