POD317 Local Anaesthesia and Surgery (8)

Abstract

This subject introduces students to the pharmacology and clinical use of local anaesthetic agents. Students will study local anaesthetics in detail to ensure a comprehensive knowledge of these agents, as preparation for the practice of cutaneous surgery. Successful completion of this subject enables students to safely administer local anaesthetic drugs in topical and injectable forms. Students will develop foundational knowledge for the practice of cutaneous surgery, with a specific focus on nail surgery and electrosurgery techniques. The theory of surgery will also encompass podiatric surgery techniques for the correction of foot deformity, the use of surgery as prophylaxis e.g. in the diabetic foot, and orthopaedic surgery for hip and knee pathology. The ethical and medico-legal implications of surgery will underpin the students' study, with a strong emphasis on client selection, surgical assessment and post-operative care. This subject will also address general surgical principles, reinforcing and expanding the students' understanding and practice of infection control, and clinical protocols.

Learning Outcomes

Upon successful completion of this subject, students should:
  • be able to explain the pharmacodynamics and pharmacokinetics of a range of amide local anaesthetic agents;
  • be able to justify the clinical use of local anaesthetics in podiatric practice;
  • be able to evaluate the indications and contraindications for the use of local anaesthetic agents;
  • be able to analyse the potential adverse effects associated with systemic administration of local anaesthetic agents;
  • be able to administer injectable amide local anaesthetic agents safely, for diagnostic and therapeutic purposes;
  • be able to explain the acute management of a range of clinical emergencies e.g. anaphylaxis, vaso-vagal episode, hyperventilation, cardiac arrest
  • be able to demonstrate safe practice in the preparation and administration of local anaesthetic agents for regional anaesthesia;
  • be able to analyse the medico-legal and ethical issues associated with surgery;
  • be able to evaluate pre, peri and post-operative management strategies;
  • be abel to demonstrate competence in infection control, universal precautions and clinical/theatre protocols relevant to surgery;
  • be abel to demonstrate competence in nail surgery procedures 'in vitro';
  • be able to justify the role of podiatric surgery techniques for the alleviation of pain and/or correction of foot deformity;
  • be able to explain a range of surgical procedures for the alleviation of pain and/or correction of foot deformity;
  • be able to evaluate client suitability for a range of forefoot surgery e.g. digital surgery, neurectomy, 1st ray surgery;
  • be able to explain the role of surgery in the management of hip and knee pathology.

Syllabus

The subject will cover the following topics:
  • Local anaesthetic agents - pharmacology, indications and contraindications, maximum safe dose calculations, adverse effects
  • Local anaesthetic agents - clinical use in podiatry
  • Local anesthetic injection techniques - local infiltration, digital block, ankle block and ray block (Mayo block)
  • Clinical emergencies and acute management e.g anaphylaxis, vaso-vagal episode, hyperventilation, cardiac arrest
  • Drugs used in the acute management of hypersensitivity reactions
  • Use of adrenaline auto-injector devices
  • Medico-legal and ethical implications relevant to surgery
  • Clinical and theatre procedures and protocols
  • Pre and peri-operative assessment including referral for diagnostic investigations
  • Post operative complications - client assessment and management
  • Principles and practice of surgery for cutaneous pathology in the foot and ankle including: nail surgery; matrixectomy techniques; electrosurgery techniques; surgical excision of intractable plantar keratoses
  • Principles of forefoot surgery techniques - digital surgery, neurectomy, 1st ray surgery
  • Principles of surgery for the correction of painful flat foot
  • Indications for and implications of orthopaedic hip and knee surgery

Work Place Learning

This subject contains a Compulsory Workplace Learning component of 1.1 days duration. Participation in local anaesthesia tutorials and practical sessions (1 day).
