This subject will enable students to develop a holistic understanding of the issues related to chronic condition self-management from a lifespan and biopsychosocial perspective of the person with chronic conditions. Through case studies, personal research and reflection on practice, students will enhance their understanding of nursing interventions used to support people with chronic conditions, their interactions with health systems and their use of self management techniques and models. Students will develop an understanding of the barriers to and facilitators of effective self-management for and by the person.
