Students will have the opportunity to synthesise the learning they have achieved in other chronic care subjects and apply this to a specific health condition or practice area of their choice. This subject enables students to extend their scope of chronic nursing knowledge and practice by investigating in detail, the specific needs and challenges of their chosen topic, in relation to nursing practice with the outcome being to inform chronic care nursing practice from a clinical, management, research or educational perspective. Students will identify, in collaboration with the subject co-ordinator and clinical mentor, a specific topic to advance their knowledge and skills. The desired result is for the student to be actively involved in a process that positively informs the nursing profession and contributes to the enhancement of nursing care for people with chronic conditions.
