HLT513 Case Management and Coordinated Care in Practice (8)

This subject will explore the context of case management practice. The broad spectrum of functions of case management, the skills required for client assessments and collaborative decision making. It will consider a range of opportunities and challenges experienced when providing person-centred care within diverse contexts including understanding concepts of vulnerability, living with a disability, culture, socioeconomic and political concerns and the geographic factors. Students will reflect on the application of practice during all stages of case management intervention. Consideration will be given to developing a culturally competent workforce that increases the capacity of clinicians or workers to engage authentically with Indigenous Australian peoples across the lifespan in various health care and practice settings.

Learning Outcomes

Upon successful completion of this subject, students should:
  • be able to identify and apply theoretical frameworks that inform case management practice;
  • be able to critically analyse the complexities that impact on person-centred care in the context of case management and demonstrate advanced skills required to comprehensively assess the needs of patients/clients;
  • be able to develop innovative and advanced methods of planning, implementing and evaluating interdisciplinary approaches to case management;
  • be able to integrate and apply Indigenous Australian cultural competence to practice;
  • be able to critique their own values and beliefs in relation to case management and evaluate its impact on practice; and
  • be able to apply professional behaviours relating to global citizenship to work effectively in transcultural contexts.

Syllabus

This subject will cover the following topics:
  • The purpose and function of case management
  • The context of practice - e.g. organisational context, socio-politico-cultural contexts, geographical locations
  • Models of healthcare in case management
  • Working with consumers/clients, families and carers, including Indigenous Australians, in complex diverse situations
  • Leading and facilitating goal-directed planning
  • Working inter-professionally and inter-sectorally
  • Managing resources and referrals
