NRS397 Health Challenges 5: Chronic Care (8)

This subject supports students to analyse and apply creative scientific problem-solving approaches to nursing care for people with chronic health conditions related to recurring and continuous health challenges. This includes examination of the pathophysiology of these chronic health challenges, recognition of clinical manifestations, and the development of appropriate nursing care to enhance patient outcomes and assure patient safety. Students are also supported to create opportunities for interprofessional contributions to the care of the person with chronic health challenges, locally, nationally and globally.

Learning Outcomes

Upon successful completion of this subject, students should:
  • be able to discuss and link the pathophysiology, clinical manifestations, and nursing interventions for people with chronic health challenges;
  • be able to evaluate the impact of multiple comorbidities on chronic health challenges;
  • be able to plan, justify, and evaluate the nursing care required for the person who receives pharmacological and non-pharmacological interventions for chronic health challenges;
  • be able to explain and justify interprofessional care for the person experiencing chronic health challenges; and
  • be able to examine frameworks and models of care for the management of chronic health challenges.

Syllabus

This subject will cover the following topics:
  • Module 1: Chronic health challenges: Definitions and common examples;
  • Module 2: Preventative measures for people with chronic health challenges;
  • Module 3: Nursing caring approach for people with chronic health conditions;
  • Module 4: Supporting people and families with chronic health conditions; and
  • Module 5: Global perspectives on chronic health conditions.
