PHS350 Integrated Chronic Health Condition Management (8)

This subject emphasises the integrated nature of physiotherapy practice in the management of chronic health conditions. Students will develop a deeper understanding of the lived experience of chronic health conditions, holistic perspectives of health and wellbeing, collaborative practice and culturally responsive approaches to chronic health care and other conditions. Students undertaking this subject will develop their skills further in general physiotherapy assessment and management, evidence-based practice, and demonstrated skills in clinical reasoning. Students will develop their understandings of person centred care through development of skills in interviewing, goal setting and physiotherapy management for people with chronic conditions living in rural and remote settings.

Learning Outcomes

Upon successful completion of this subject, students should:
  • be able to demonstrate and articulate sound clinical reasoning processes which analyse assessment findings in relation to physiotherapy for chronic health conditions;
  • be able to plan and implement person centred, ethical, evidence based, collaborative, safe and effective individual and group physiotherapy, for people with chronic health conditions;
  • be able to recognise the complex and interrelated factors that may impact on the client, their needs and response to physiotherapy; including for clients in rural and remote settings;
  • be able to recognise situations outside their scope of practice; and describe and justify appropriate ways in which to respond to these situations;
  • be able to reflect on their performance in a manner that demonstrates professional growth; and
  • be able to demonstrate effective and professional communication to interact with patients/clients and other health professionals.

Syllabus

This subject will cover the following topics:
  • Chronic health condition management principles such as collaborative practice, behaviour change and self-management;
  • Management strategies used in physiotherapy for chronic health conditions such as client education, group rehabilitation programs, therapeutic exercise, interviewing techniques, goal setting and E- health (for example, Telehealth);
  • Interrelated factors that impact on physiotherapy management such as social, economic, physical, historical, political, ethical, environmental and cultural considerations;
  • Physiotherapy for people with chronic conditions in rural and remote contexts;
  • Professional and ethical behaviour in the context of working with people with chronic conditions; and
  • Transitions in chronic health condition management.
