Animal Care & Ethics Committee: Guide to Using Forms
The forms must be completed on a word processor for the following reasons:
- In all Animal Care and Ethics Committee (ACEC) research applications and review forms there are questions which require answers that vary in length depending on the individual research project. Consequently the ACEC has not designated a set amount of space for answers to these questions. In addition Tables will require insertion of the appropriate number of lines.
- The ACEC has, in the past, found it difficult to decipher some hand written research application or review forms.
Complete all sections. On completion, application and review forms should be sent to:

The Executive Officer receipts applications and forwards them onto the Academic Secretariat for inclusion on the next ACEC meeting agenda. The Executive Officer notifies the research applicant of the outcome following the meeting.
ACEC Forms
All forms must be submitted typed. Response boxes are designed to expand to accommodate the information supplied by the applicant.

| ACEC-W1 | Wildlife Research Application Form | Modified ACEC-R1 to suit wildlife studies in the field (especially survey type studies). |
| ACEC-R2 | Animal Research Review Form and Application for Continuation of Research | Review form for reporting to the ACEC on research approved as per forms ACEC-R1 and ACEC-W1. |
| ACEC-T1 | Applications for the Use of Animals for Teaching Purposes | General purpose application form for the use of animals for teaching purposes. |
| ACEC-F1 | Practical Class Fieldwork: Proposal for Animal Use | Application form for teaching purposes (eg survey techniques). |
| ACEC-T2 | Teaching Review Form (including Fieldwork) | Review form for reporting to the ACEC for projects approved on forms ACEC-T1 and ACEC-F1. |
