The Mind and Body Care Navigators Project aims to develop a model of care for physical health care navigators to work within a multi-disciplinary community-based mental health team.
To our knowledge, this is the first research project of its kind in the world.
The Mind and Body Navigator Model of Care outlines the evidence-informed, lived experience-led, co-designed framework for implementing Physical Health Care Navigators within community mental health services.
It provides the principles, functions, governance, partnerships and implementation guidance needed to support better physical health outcomes, and better mortality, for people living with mental health conditions. (Funded by NHMRC TCR Grant ID 2025179, led by Prof Russell Roberts, CSU).
