Medical certificates and reports

You may need to provide a medical certificate or report when you apply for special consideration, extensions or other support.

To help us assess your request without delay, make sure your documents include all the required information.

Certificate options

Option 1

Use the Charles Sturt medical certificate

This is our recommended option.

Download and take the Charles Sturt University Student Medical Certificate form to your medical practitioner. This form guides them to include everything we need, so it's the easiest way to make sure your certificate meets requirements.

Option 2

Use a different medical certificate

You can also submit a certificate from your own medical practitioner, as long as it includes:

  • how your condition affects your studies
  • how long the condition lasts
  • how the information was provided (assessment or based on what you told them)
  • the medical practitioner's name and signature
  • a file format other than Word (for example, PDF or image)

Medical reports

If we ask you to provide a medical report, it must be a signed statement from a qualified, registered health practitioner. The report should explain:

  • how your chronic (ongoing) condition affects your ability to study
  • how long the condition is likely to last

The report should include the name of the condition, unless that would breach your privacy.
